Case Management
Mental health case management is a coordinated approach to helping individuals with mental health conditions access the care, support, and resources they need to live healthy, productive lives.
Case management services can include (but are not limited to):
- Intensive community-based services to maximize access to community resources and functioning in the community setting
- Facilitating linkage with services, coordinating integrated services from multiple providers, and monitoring progress
- Supportive coaching to increase problem solving
- Assisting with obtaining resources: transportation, housing, financial assistance, Medicaid/Medicare, SNAP, disability, etc
- Teaching skills to family members to promote positive functioning
Our Case Managers may provide support within in the community, as appropriate. Case management services are available in-person in Champaign and Bloomington and via telehealth for clients residing throughout Illinois. Priority is given to clients who are receiving other mental health services at Evergreen Coaching & Counseling.

Academic Coaching
Academic Coaching is a collaborative process where students work with a trained coach to build essential academic skills. Our coaching focuses on personalized strategies that help students stay organized, manage their time effectively, and overcome challenges—ultimately empowering them to achieve their educational goals.
Whether you’re struggling with motivation, facing test anxiety, or simply want to optimize your study habits, academic coaching can help you develop a proactive and growth-oriented mindset.

Sabrina Jones is an experienced case manager, bringing years of expertise in mental health and crisis support. She earned her Bachelor’s degree in Sociology from the University of
Arizona and has dedicated her career to empowering individuals through compassionate, client-centered care.
Sabrina provides care grounded in individualized goals and strategies designed to support each client’s unique needs. She collaborates closely with counselors, therapists, and other
professionals to deliver comprehensive, coordinated support that promotes lasting progress and well-being. Her previous work with agencies serving underprivileged communities honed her
skills in advocacy and connecting clients with essential resources.
Guided by her motto, “Meeting clients where they are, and working together to get to the next destination,” Sabrina is
deeply committed to helping every client achieve stability, growth, and lasting well-being.

Cathy Lewis, MHP

Cathy holds a Bachelor’s degree in Sociology from the University of Nebraska at Lincoln and she is excited to be a part of the new Case Management program offered by Evergreen.
Cathy has professional experience supporting children and families involved in the child welfare system, as well as working with individuals experiencing mental health hospitalization and incarceration.
Cathy brings a compassionate, person-centered approach to her work, grounded in an understanding of complex social systems and the needs of vulnerable populations.
She is ready to help connect you with tools and resources to help you be successful.
