Case Management

Mental health case management is a coordinated approach to helping individuals with mental health conditions access the care, support, and resources they need to live healthy, productive lives.

Case management services can include (but are not limited to):

  • Intensive community-based services to maximize access to community resources and functioning in the community setting
  • Facilitating linkage with services, coordinating integrated services from multiple providers, and monitoring progress
  • Supportive coaching to increase problem solving
  • Assisting with obtaining resources: transportation, housing, financial assistance, Medicaid/Medicare, SNAP, disability, etc
  • Teaching skills to family members to promote positive functioning

Our Case Managers may provide support within in the community, as appropriate. Case management services are available in-person in Champaign and Bloomington and via telehealth for clients residing throughout Illinois. Priority is given to clients who are receiving other mental health services at Evergreen Coaching & Counseling.

Academic Coaching

Academic Coaching is a collaborative process where students work with a trained coach to build essential academic skills. Our coaching focuses on personalized strategies that help students stay organized, manage their time effectively, and overcome challenges—ultimately empowering them to achieve their educational goals.

Whether you’re struggling with motivation, facing test anxiety, or simply want to optimize your study habits, academic coaching can help you develop a proactive and growth-oriented mindset.
